CCDD – IDOT 2290 Forms

For our customers whose projects are awarded by the Illinois Department of Transportation (IDOT), Article 107.22 of the Standard Specifications for Road and Bridge Construction requires contractors to seek the Department’s approval of borrow, use and waste sites prior to their use in a construction project. This “Waste/Use Area Review” form must be completed and submitted to IDOT prior to any acceptance of materials at the site* and can be found here for your convenience:

Typically, additional maps and drawings are required in conjunction with this form and are the responsibility of the applicant. This BDE 2290 form also includes the Landowner Agreement, which must be filled out and signed by the appropriate owner(s). * Please note that these forms do not guarantee acceptance of the depository materials or stamped IDOT approvals. All items must be free of contaminants and all loads are inspected and tested prior to placement. Sexton reserves the right to refuse any load at their sole discretion.
